Chronic Disease Self-Management Program Workshop Conducted in Ulithi

Yap FM - On Thursday, September 3, 2026, Yap’s Non-Communicable Disease (NCD) Program Coordinator, Agatha Legasiuggiy, traveled to Ulithi with National FSM Diabetes Program Coordinator Selma Primo and FSM Tobacco Program Coordinator Shellany Saimon to conduct a three-day Chronic Disease Self-Management Program (CDSMP) workshop.


The workshop was held at the dispensary in Falalop and brought together 12 NCD patients and family caregivers. Ten participants successfully completed the full three-day training. The team returned to Yap on Monday, September 7.


CDSMP is an evidence-based, community-focused program developed by Stanford University that helps people living with chronic conditions and their caregivers develop practical skills to better manage their health. The program normally runs for six weeks, but the Ulithi training was condensed into three days to meet the needs of the community.


Participants learned practical strategies for managing chronic disease symptoms, healthy eating, appropriate exercise, medication use, communication with health care providers, stress management, action planning, problem solving, and decision making.


The workshop also emphasized setting and achieving small, realistic health goals to build confidence and encourage healthier, more active lifestyles.


The Ulithi workshop is part of ongoing efforts by Yap State and FSM health programs to bring NCD education and self-management support directly to patients and families in the outer islands of Yap.
